This Notice describes how Compass Cognition, LLC may use and disclose your protected health information (PHI) and how you can exercise your rights regarding that information. Please review it carefully.

Our Legal Duties

Compass Cognition, LLC is required by law to maintain the privacy of your protected health information, to provide you with notice of our legal duties and privacy practices, and to notify you in the event of a breach of unsecured protected health information. We are required to abide by the terms of the Notice currently in effect.

Special Protections for Substance Use Disorder Records

Some health information we maintain may relate to substance use disorder (SUD) diagnosis, treatment, or referral for treatment. This information may be subject to additional federal confidentiality protections under 42 C.F.R. Part 2, which provides greater privacy protections than HIPAA for certain records. When these additional protections apply, we may be more limited in how we use or disclose this information, even for treatment, payment, or health care operations, unless you provide written consent or another legal exception applies.

You have the right to an accounting of disclosures of Part 2 records and the right to request restrictions on certain disclosures of Part 2 records. You may also file complaints about Part 2 violations directly with the Secretary of the U.S. Department of Health and Human Services.

Marketing and Sale of Information

Compass Cognition, LLC will not use or disclose your protected health information for marketing purposes or sell your information without your written authorization.

Changes to This Notice

We reserve the right to change the terms of this Notice at any time. Any revised Notice will be effective for all health information we maintain. A current version of this Notice will be posted on our website and available at our office.
